A plain guide for nurses to record shift handoff summaries, with the steps, tools to try and mistakes to avoid.

Record what happened in the shift, what still needs doing and what the next nurse must know, while the details are still fresh, in the place your unit uses for handoff.

| Option | How it captures the summary | Card fee |
|---|---|---|
| Pen and the handoff form | You write each key point in the form's order | not published |
| A note in the unit's chart or document system | You type the summary where the next nurse looks | not published |
| A voice memo app on a phone | You speak the summary and play it back before typing it up | not published |
| QuickDictate | Types what you say into whichever window has focus [^1] | free for personal and other noncommercial use [^1] |

## What you need before you start

Recording a shift handoff summary takes only a few things.

- **A clear idea of what belongs in it.** Most units want the care given, what still needs doing and anything the next nurse must watch for.
- **The facts fresh.** Write or record the details soon after care so nothing important is missed.
- **A place to keep it.** A handoff form, a note in the chart or a document in the system your unit uses.
- **A way to get the words down.** Typing, writing or speaking all work; the point is to capture the summary while it is accurate.
- **Time set aside before handoff.** A short block at the end of the shift, so the summary is ready when the next nurse arrives.
- **Your unit's rules.** Check how and where summaries may be kept, and what patient information they may contain.

Set aside a calm minute before handoff to check the day's notes so nothing is missed.

## Step by step

These steps work with a pen, a chart entry or a spoken summary.

1. **Start while the details are fresh.** Begin the summary during or just after care, not at the end of a long shift.
2. **Note the essentials.** Name the patient, the key events of the shift and the results the next nurse needs to see.
3. **Say what is left.** Procedures, medications or observations still due before or during the next shift.
4. **Add what the next nurse must know.** Anything to watch, any concern and any change in the care plan.
5. **Follow the unit's format.** Where a form exists, keep its order so nothing gets skipped.
6. **Say it plainly.** Short sentences and concrete facts, with no guesses about what happened.
7. **Read it back once.** Catch errors or gaps while there is still time to fix them.
8. **Store it where the unit expects.** In the chart, the handoff system or the agreed place.
9. **Hand it over.** Give the next nurse the summary and answer any question they raise.
10. **Keep a copy safe if the unit requires it.** Know the rule before handoff, so the summary lives somewhere it can be found later.

## Tools that can do it

You do not need software for this. Here are options to consider.

**Pen and the handoff form.** You write each key point in the order the form asks for. Nothing is published here about price or limits because it depends on what your unit already supplies.

**A note in the unit's chart or document system.** You type the summary where the next nurse will look for it. Check your unit's system for its own limits and for how long notes are kept.

**A voice memo app on a phone.** You speak the summary and play it back before you type it up. Facts about specific voice apps are not provided here, so check their own pages for cost and limits, and keep patient details in line with your unit's rules.

**QuickDictate.** QuickDictate is made by the team that publishes this site. It is one option. QuickDictate is a Windows program that types what you say into whichever window you are using. [^1] You hold a key while you talk, or press it once to start and once to stop, and you can change both keys. [^1] What you say is typed into whichever window has focus, such as an editor, chat box, terminal or web form. [^1] You can use ElevenLabs, Deepgram, OpenAI, AssemblyAI, DashScope or Google, and that provider bills you directly. [^1] Dictation can run on the computer with no internet connection or API key once a speech model is installed. [^1] With five of the cloud providers, words appear while you are still talking. [^1] You can list words and phrases that are sent to the provider to help it recognise them, and a replacement table fixes spellings such as "Chat GPT" to "ChatGPT". [^1] Profiles can change the provider, language, vocabulary and formatting depending on which app is in focus. [^1] The last 50 dictations can be browsed and filtered in Settings, and the history survives a restart or self-update. [^1] Saying "scratch that" discards the text you just dictated instead of inserting it. [^1] While you are dictating, other apps' audio is muted or turned down, and it goes back to its old level when you stop. [^1] If you sign in, preferences such as hotkeys and language can be synced between PCs. [^1]

QuickDictate is free for personal and other noncommercial use, with no subscription or account. [^1] Cloud providers bill you separately for their service. [^1] From v0.9.0 on, it is source-available under PolyForm Noncommercial 1.0.0, and commercial use needs a separate licence. [^1] It runs only on 64-bit Windows 10 and 11. [^1] It is delivered as an .exe file on GitHub, with a built-in updater that works silently. [^1] Check that the unit's computers run Windows 10 or 11 before relying on it.

## Common mistakes to avoid

**Recording the summary too late.** Waiting until the very end of the shift makes it easy to forget a change or a result. Start while the details are fresh.

**Leaving out what still needs doing.** Finish the summary only when the jobs and observations left for the next shift are in it.

**Giving opinions instead of facts.** Say what you observed and what happened, and leave out guesses about the patient's state.

**Keeping the summary where it cannot be found.** Use the place your unit agrees on, or the next nurse will not see it in time.

**Skipping the read-back.** A quick check catches a missing medicine, a wrong result or a name spelled wrongly.

**Not noting a change in condition.** If the patient's state changed during the shift, say so plainly at the top of the summary.

## Frequently asked questions

### What should a nurse's shift handoff summary include?

The care given during the shift, the patient's condition, what still needs doing and anything the next nurse must watch for. Most units also want the key results and any change to the care plan. Follow the unit's format where one exists and keep the summary concrete. Record it while the details are fresh so nothing important is missed.

### When should a nurse record the shift handoff summary?

Record it while the details are still fresh, during or just after care. Waiting until the very end of the shift makes it easy to forget a change or a result. Start the summary early and finish it before handoff, so the next nurse gets a complete picture and can ask any question while you are still there.

### Can QuickDictate type a handoff summary into the chart?

QuickDictate types what you say into whichever window has focus, such as the chart, a note or a form. [^1] You hold a key while you talk, or press it once to start and once to stop. [^1] It runs only on 64-bit Windows 10 and 11, so check that the unit's computers meet this. [^1]

### Does QuickDictate cost anything for nurses?

QuickDictate is free for personal and other noncommercial use, with no subscription or account. [^1] Cloud providers bill you separately for their service. [^1] It is free for personal and other noncommercial use, and commercial use needs a separate licence. [^1] Check which use applies to your workplace before relying on it.

### Does QuickDictate work on every computer?

No. It runs only on 64-bit Windows 10 and 11. [^1] Check that the unit's computers run one of those before relying on it for handoff summaries. Where a computer does not meet this, a pen and the handoff form or a note in the unit's system are simple alternatives that need no extra software at all.
